Required Paperwork for Summer Camp

Below are the forms we require for your child at summer camp. Ideally, these forms are uploaded into Active Network one week prior to Sunday Check-in. You can submit them by uploading them onto your online account, through email, or if mailed in plenty of time to arrive one week before camp by snail mail (Stone Canyon Outdoor EdVentures, 12163 S Perry Park Road, Larkspur, CO 80118.)

Please save a copy of all forms and bring them to check-in, even if previously submitted.

  • Mandatory for all OVERNIGHT Summer Campers

  • Must be signed by a Physician or approved Health Care Provider and CURRENT (within the last 12 months)

  • Mandatory for ALL Stone Canyon Summer Campers (Overnight and Day)

  • State law determines that we can only accept the Colorado Certificate of Immunizations form only. This form must be filled out and signed by a physician (for example, you cannot attach a different form and write “see attached”, the Colorado form itself must be filled out by the physician.)

  • Mandatory for Campers bringing any type of medication (e.g. prescription, OTC, vitamins, etc. Note: please do not send homeopathic and/or essential oils with your child to camp.)

  • Must be signed by a Physician or approved Health Care Provider

  • Medications and supplements must be in their original packaging with dosage clearly labeled at check-in
